Omni Space Float Service – Standard Waiver and Consent Form

Thank you for choosing Omni Space. We offer Float Therapy services (known as the Service). These terms and conditions (Terms) relate to your participation in the Service. By making a booking for the Service and/or attending Omni Space, you indicate your acceptance of these Terms. Please thoroughly read the terms below and complete the relevant information required below:

1. Health and safety

Every guest fills in a health questionnaire before their first visit, for every service. Completing it fully and truthfully is the guest’s responsibility. Please also tell us if your health changes. We rely on what you declare, and we are not responsible for consequences arising from missing or inaccurate answers. If you have a medical condition, are pregnant or take medication, please ask your doctor before you come.

Please do not use the float pool (tell reception and we will reschedule) if you:

• have had alcohol or recreational drugs in the last 12 hours;
• have a contagious skin condition, an open wound, or a tattoo or piercing that has not healed;
• have dyed, bleached or chemically treated your hair in the last 48 hours, or had a spray tan in the last 6 days (to keep the water clean for everyone);
• are 36 or more weeks pregnant;
• are menstruating;
• have bladder or bowel incontinence;
• have a history of epilepsy, seizures, fainting or heart disease, use a pacemaker or defibrillator, or have kidney disease or uncontrolled diabetes, without your doctor’s approval;
• have a condition that may be affected by magnesium absorbed through the skin.

Shaving or waxing in the 24 hours before a float may make salt water sting.

Sauna and cold plunge are not suitable if you have a fever, have been drinking, have heart disease, uncontrolled blood pressure, Raynaud’s syndrome, or are pregnant, unless your doctor agrees. Drink water, leave the room straight away if you feel dizzy, and follow staff instructions at the cold plunge. Brainwaves Neuro Align is not for people with epilepsy or sensitivity to flashing light.

You may stop a session at any time. Staff may pause or decline a service if it seems unsafe for you or others; we will then reschedule with you.

2. Responsibility

After deep relaxation, heat or cold, some people feel mild dizziness, nausea, a headache or tiredness. This is usually temporary; if it persists, please see a doctor. You are responsible for risks that arise from inaccurate health information or from not following staff instructions. Ōmni Space is responsible, as Vietnamese law provides, for harm caused by our fault. Nothing in these terms reduces your rights as a consumer under the law.

3. Personal data

We collect your name, contact details, health questionnaire, signature and (when needed) invoice details to manage bookings, keep you safe, issue invoices and look after you. Health information is kept confidential and seen only by the staff who need it. We do not sell your data; we share it only with payment, email or booking providers when needed to deliver the service, and when the law requires. You can ask to see, correct or delete your data at info@omnispacevn.com, in line with Vietnamese personal data protection law.

GUEST ACKNOWLEDGEMENT

• I have completed the health questionnaire myself, fully and truthfully, and I understand this is my responsibility.
• None of the situations above apply to me (or my doctor has approved), and I will tell the team at once if my health changes.
• I will follow staff instructions, and I understand that Ōmni Space services are not medical services.
• I have read and agree to the Ōmni Space Terms and Conditions.
